MyChart is a website and a free mobile app that is free to use. It offers patients online access to parts of your medical record. It is a safe and secure way to manage and receive info about your health. You can use MyChart to:
- Request visits.
- View your health summary.
- View test results.
- View doctor’s notes about your care.
- Request medicine refills.
- Access trusted health info.
- Send and receive online messages from your medical care team.
Patients who want to use MyChart will be issued an activation code during a visit. You will need this code to log in and create your own account. If you do not have a code, you may ask to sign up at your next visit.
For your security, the code expires after 14 days. It also stops working after the first time you use it. You can request a new code at your next clinic or hospital visit.

The website for MyChart is in English and Spanish. Once you log in, your medical data may be in English only.
Yes, you can. This is called proxy access and allows another person to view your health info on MyChart. You will need to sign a form to allow them to have this access. Parents or legal guardians may also request access to a child’s medical record.

You can also change your mind and remove proxy access at any time.
Almost all lab test results are sent to your MyChart account once they are ready. Some tests might take up to 5 days to be posted to MyChart; these include results for HIV, Hepatitis, and a few others.

You will usually get an answer to your MyChart message within 3 business days.
Please note that MyChart should NOT be used for urgent messages. Please contact your care team directly if you need immediate attention. Always call 911 if it is an emergency.
We work hard to make sure your health record is kept private and secure. MyChart uses activation codes, personal usernames, and passwords. Each person controls their password, and the account cannot be accessed without that password. All messaging is also done while you are safely logged in.
MyChart follows federal and state privacy laws. Your name and e-mail address will be treated with the same care and privacy given to your health records. Your info will never be sold or leased by MyChart. The full San Francisco Department of Public Health’s Privacy Policy is available here.
You will be logged out of MyChart if you are inactive for more than 15 minutes. We suggest you log out of MyChart if you need to leave your computer, tablet or cellphone for any reason.
